We offer, free of charge, two types of informational cards about Chemo Angels. These are the same cards we send to oncologists, treatment centers, and cancer support groups. If you would like to receive 15 cards, please submit the request form below. (If you request both types of cards, you will receive 8 of each.)

PLEASE do not order cards unless you are sure you will use them! 

Unfortunately, at this time we can only send to addresses in the United States.
